Position Summary
We are seeking a compassionate and dedicated Licensed Master Social Worker (LMSW) to provide clinical and supportive services to individuals and families experiencing homelessness. The LMSW will work directly with clients to assess their needs, develop individualized service plans, connect them with community resources, and provide ongoing counseling, crisis intervention, and case management support.
The ideal candidate will have experience working with individuals experiencing homelessness, mental health concerns, substance use, poverty, trauma, and other barriers to stable housing and self-sufficiency.
Essential Duties and Responsibilities
- Conduct comprehensive psychosocial assessments and identify clients' immediate and long-term needs.
- Develop and implement individualized service plans in collaboration with clients.
- Provide individual supportive counseling, crisis intervention, and emotional support.
- Assist clients with identifying and addressing barriers to obtaining and maintaining permanent housing.
- Connect clients to community resources, including housing, healthcare, mental health services, substance use treatment, benefits, employment, food assistance, and other social services.
- Advocate on behalf of clients with government agencies, healthcare providers, housing providers, and community organizations.
- Assist clients with applications for public benefits, housing programs, entitlements, and other supportive services.
- Monitor client progress and regularly update service plans based on changing needs.
- Provide intervention and support during mental health or other psychosocial crises.
- Participate in case conferences, team meetings, and interdisciplinary collaboration.
- Maintain accurate, timely, and confidential documentation in accordance with agency policies and regulatory requirements.
- Complete required assessments, progress notes, referrals, incident reports, and other clinical documentation.
- Maintain professional relationships with clients while establishing appropriate boundaries.
- Participate in outreach and engagement efforts to connect hard-to-reach individuals with services.
- Coordinate discharge planning and facilitate successful transitions to permanent housing and/or community-based services.
- Collaborate with housing specialists, case managers, medical providers, mental health professionals, and other members of the care team.
- Maintain knowledge of available housing and social service resources within the community.
- Follow all applicable agency, HIPAA, confidentiality, and mandated reporting requirements.
- Perform other duties as assigned.
Qualifications
- Master's degree in Social Work (MSW) from an accredited social work program.
- Current LMSW license required.
- Experience working with individuals experiencing homelessness, housing instability, or other vulnerable populations strongly preferred.
- Experience providing case management, counseling, crisis intervention, or supportive services preferred.
- Knowledge of NYC/State housing and social service systems is a plus.
- Strong understanding of trauma-informed care and harm-reduction principles.
- Excellent communication, interpersonal, organizational, and documentation skills.
- Ability to work effectively with individuals from diverse backgrounds and with complex psychosocial needs.
- Ability to work independently while also functioning effectively as part of a multidisciplinary team.
- Strong advocacy and problem-solving skills.
- Proficiency with electronic case management and/or electronic health record systems preferred.
