Case Manager - School Services
Job Description
NWON is looking for motivated, caring, individuals with integrity and compassion for children and community. The individuals with these qualities coupled with social services qualifications will be hired for two full time Schools Case Manager roles. This role will focus on services that ensure household stability. These services include but are not limited to providing direct connections to
resources for parents and families, hosting Family Engagement Forums at the school and
nearby for the school community, and working with other social service professionals to
better support the School Advisory Council and survey and needs assessment processes.
Schools Case Managers support families navigating social supports,
socio-economic programs, health supports, and school supports to maintain
school attendance and foster positive school outcomes. Must pass child abuse, PA State and national background clearances. Must pass a drug screening.
Responsibilities
Receive referrals from Community School stakeholders or families that
have an identified attendance, social, economic, behavioral or health
need that is or may prevent the student from attending and being successful in school.
• Provide support services or Family Forums based on the identified needs
of the school community
• Make efforts to engage the families during the entire referral process and gain
access to the home.
• Conduct all required ongoing home visits in the families’ own home. Remain in
close contact with families during the entire service period.
• Create all required service plans based on assessed needs and barriers.
• Support children’s health and well-being.
• Provide access to Emergency Funds.
• Contribute to the climate of supporting attendance by providing
incentives and facilitating attendance events in conjunction with
Community School Coordinator.
• Ensure families are aware of and can access services provided through
Community Schools and other partners.
• Attend appointments and meetings with clients to help families access services,
• Attendance case manager will participate in school based MTSS/Attendance
meetings to identify students that need support
• Attendance case manager will work to remove the barriers to attending school,
leading to fewer accrual of absences
• Provide referrals to address a wide array of case management services including
but not limited to social services, behavioral services, domestic violence services,
• housing resources, primary care services, and economic resources.
• Conduct regular phone contact and visits with assigned families.
• Maintain consistent contact with school personnel and participate in school
meetings.
• Maintain proper documentation for all cases.
