Intake Specialist

• Conduct initial client intake interviews and assessments

• Develop a therapeutic alliance with clients

• Educate individuals entering the program about Serenity's services and provide a detailed overview of Patient Rights and responsibilities within the residential program

• Complete all necessary consent forms with clients

• Prepare patient record for distribution to primary counselors

• Provide individual substance abuse counseling to a partial caseload

• Facilitate group therapy, individual counseling, and teaching to a partial caseload as needed

• Monitor Serenity's referrals

• Use the ASAM and SBIRT assessments

• Complete all necessary paperwork and checklists of regulations and agency standards

• Upload admission documents to the data system

• Utilize ASAM during intakes to determine level of care

• Participate in training opportunities

• Participate in case conferences and weekly team meetings

• Initiate communication with relevant referral sources

• Collaborate with Parole Officers to determine placement

• Collaborate with the Transportation Coordinator to ensure transportation to CBP program placement

• Assist the Community Service Representative in collaborating with a participant's Parole Officer to determine placement

• Develop, implement, and evaluate individualized treatment plans

• Collaborate with multidisciplinary team, community professionals, and community agencies to ensure continuity of care

• Complete all assignments and any other duties assigned by the Clinical Director

• Maintain timely clinical documentation in compliance with program policies and regulatory standards
