Dr. Mae Sheikh Ali Endocrine Center in Jacksonville, FL is looking for a compassionate and dedicated Licensed Clinical Social Worker (LCSW) to join our specialized medical team. Responsibilities:
- Provide individual and group therapy to patients managing chronic endocrine disorders such as diabetes, thyroid disease, and hormonal conditions
- Conduct psychosocial assessments and develop individualized treatment plans
- Collaborate with physicians, nurses, and care teams to deliver integrated, whole-patient care
- Offer crisis intervention, counseling, and mental health support to patients and families
- Connect patients with community resources, support groups, and social services
- Maintain accurate clinical documentation in compliance with HIPAA and regulatory standards
- Participate in care coordination meetings and multidisciplinary team discussions
Requirements:
- Active Florida Licensed Clinical Social Worker (LCSW) license required
- Master's degree in Social Work (MSW) from an accredited program
- Experience in a clinical, medical, or healthcare setting preferred
- Strong knowledge of psychosocial assessment, crisis intervention, and evidence-based therapy
- Excellent communication and interpersonal skills with a patient-centered approach
- Ability to work collaboratively within a multidisciplinary medical team
- Proficiency with electronic health records (EHR) systems
