Place of Performance Services will be performed in the Health Services Department of the FCI Oxford located at 350 Elk Avenue Oxford, Wisconsin 53952.


Schedule-The contractor is to provide one pharmacist to work 40 sessions per week. One (1) session will equal one (1) hour. An anticipated schedule for the provision of pharmacist services is Monday through Friday working 8 hours per day, between the hours of 6:00am to 4:00pm, excluding federal holidays. This anticipated schedule includes a 30-minute non-paid lunch break. Session lengths are defined as one (1) hour.


Description of Duties-

The contractor shall provide pharmacist services which will include, but are not limited to:

•Serves as a resource person to the medical providers in the field of medicine as it relates to proper pharmacy practice, patient profiles, and review of prescriptions written. •Conveys proper pharmacy procedures and practices to medical staff via written memorandums, direct personal contact, and through personal observation. •Monitors drug therapy for contraindications, interactions, allergies and appropriateness .•Maintains patient medication records to include medication profile formats.•

Responsible for the documentation of work performed.•Familiarization with administrative guidelines of the Joint Commission on the Accreditation of Health Care Organizations, Drug Enforcement Administration, Food and Drug Administration and Bureau of Prisons. •Provides a balanced and efficient pharmacy service, establishes and maintains standards of service, and maintains maximum efficiency in economy of operations. •Incumbents make independent decisions in the performance of duties and as such makes recommendations for revision or improvement as appropriate. •Responsible for the selection, dispensing, and preservation of drugs, medicines, and other therapeutic agents consulting updated references, journals, reports, etc. •Required to identify and ascertain differences between drugs as it relates to the therapeutic efficacy of the product. •Periodically inspects drugs for adequacy, overstocking, arrangement, storage, proper rotation and deterioration. •Assists in maintaining an inventory of commonly utilized drugs and assumes responsibility for security of drugs in his/her custody .•Uses the proper techniques for the storage of drugs, such as refrigeration or protection from heat and light. •Obtains familiarity of the Bureau of Prison’s National Drug be alert to avoid any possible changes which may result in deterioration of the product and loss of its potency, formation of toxic products, or other undesirable effects in addition to adverse patient care. •Provides a wide range of drug information to medical staff.•

Maintains adverse drug reaction (ADR) reporting system by investigating events to determine if an ADR has occurred. •Incumbent is available to the medical staff relative to consultations of the patient treatment aspects of medication as it relates to drug compatibility, mode of action, drug reactions, latest developments in the field of pharmacy. • Review of medication orders prescribed by the medical staff, this is accomplished through individual consultations with the medical staff and review of medical records as indicated.

Professional Qualifications/Credentialing/Certification

The contractor shall possess the necessary skills, abilities, and credentials to perform professional Pharmacist(Pharm D) services. The Contractor shall have current certification that permits him/her to practice and provide Pharmacy services in the State of Wisconsin for the duration of the BPA. The contractor must identify the individual selected to perform these services on behalf of the contractor at the time the quotation is submitted. Failure to submit evidentiary documentation (resume) of education, qualifications, and certifications or the name of a qualified candidate who will perform these services may result in elimination from consideration for award. Copies of the following shall be submitted with quote.

•Valid and current License that allows for the practice of Pharmacy (Verified). •Proof of CPR certification.
