Schedule: Monday-Friday, 8:30 AM-5:00 PM
Practice Setting: Closed-door pharmacy
On-Call: Rotating weekly on-call schedule, including weekends
We are seeking an experienced Clinical Pharmacist to join a closed-door pharmacy team in Louisville, KY. This role is ideal for a detail-oriented and collaborative pharmacist with strong clinical pharmacy experience and an interest in nutrition support and home infusion services.
The pharmacist will provide clinical oversight for patients receiving specialized nutrition support, including parenteral nutrition, while collaborating with healthcare professionals, patients, caregivers, and pharmacy staff to promote safe and effective patient outcomes.
Key ResponsibilitiesProvide clinical nutrition support information and consultation to healthcare professionals.
Review prescriptions for accuracy, including formulas, ingredients, medications, and administration instructions.
Monitor patients receiving parenteral nutrition by reviewing medical records and clinical information.
Identify potential metabolic complications and take appropriate action to address patient needs.
Provide guidance regarding drug interactions, side effects, dosage, storage, and medication-related concerns.
Respond promptly to patient, caregiver, and healthcare professional questions and concerns.
Oversee pharmacy staff involved in sterile compounding and dispensing activities.
Verify final compounded sterile preparations and related supplies for accuracy and completeness.
Ensure accurate and timely documentation of patient information, medications, clinical assessments, and communications.
Maintain compliance with pharmacy policies, procedures, and applicable regulations.
Participate in department meetings and quality improvement activities.
Bachelor's or Doctorate of Pharmacy (PharmD) from an accredited university.
Minimum 2 years of clinical pharmacy experience required.
Current, active Pharmacist license in the applicable state.
Additional state pharmacy licenses may be required based on operational needs.
Continuing education must be maintained in accordance with applicable state requirements.
Strong clinical judgment and problem-solving skills.
Excellent communication and interpersonal skills.
Ability to interpret prescriptions, clinical information, technical procedures, and regulatory requirements.
Strong mathematical skills, including the ability to calculate formulas, proportions, percentages, ratios, and volumes.
Ability to work effectively both independently and as part of a multidisciplinary team.
Previous experience in nutrition support, parenteral nutrition, home infusion, or sterile compounding is required.
