The Occupational Therapist will deliver services in both clinic and in-home settings, working as a key member of an interdisciplinary care team. This role focuses on maximizing patient safety, independence, and quality of life.Position Details:Job Title: Occupational TherapistLocation: Stockton, CaliforniaSchedule: Full-Time, Monday to FridayEmployment Type: PermanentKey Responsibilities:Conduct comprehensive Occupational Therapy assessments to determine patient needs, goals, and level of functionDeliver and document therapeutic interventions including perceptual-motor training, cognitive function support, ADL re-education, balance, endurance training, and use of adaptive equipmentCollaborate with physicians and interdisciplinary team members to develop and update individualized care plansSupervise Occupational Therapy Assistants, including training and guidance on Functional Maintenance Programs (FMPs)Educate caregivers, patients, and staff on therapy techniques and use of assistive devicesSupport smooth transitions of care between home, hospital, and facility settingsParticipate in end-of-life care planning and support when neededRequirements:Bachelor's degree in Occupational Therapy from an AOTA/ACOTE-accredited program (Master's preferred)Minimum of two (2) years of clinical Occupational Therapy experience with elderly or medically complex populationsTwo (2) years of supervisory experience preferredRegistered with the National Board for Certification in Occupational Therapy (NBCOT)Active California OT license or eligibility for licensureCPR certification preferredBenefits:Medical, dental, and vision insurance17 days of PTO, 12 paid holidays, and sick leave401(k) with employer matchBonus and equity opportunitiesCollaborative and mission-driven team culture
