About the Role:
We are seeking a highly skilled and dedicated Licensed Practical Nurse (LPN) to join our team at Mountain View Rehabilitation and Senior Living Center in Bloomsburg, Pennsylvania. As an LPN, you will play a vital role in providing high-quality patient care while collaborating with our interdisciplinary team to achieve exceptional outcomes.
Responsibilities:
• Provide skilled nursing care to patients in accordance with established policies and procedures, physician orders, and patient needs
• Conduct initial and ongoing assessments of patients' physical and emotional needs
• Develop and implement individualized care plans in collaboration with the interdisciplinary team
• Administer medications and treatments as prescribed by physicians
• Participate in patient education and discharge planning
• Maintain accurate and detailed records of patient care
• Collaborate with other healthcare professionals to ensure comprehensive care

Requirements:
Current LPN license in the state of Pennsylvania
High school diploma or equivalent required; associate's or bachelor's degree preferred
