In this role, you will provide personal care, assistance, supervision, and companionship to residents. You will help residents maintain their independence, dignity, comfort, and quality of life while following individualized service plans and direction from the licensed nursing team.

What You'll Do

  • Provide respectful and compassionate personal care to residents
  • Assist residents with activities of daily living, including bathing, dressing, grooming, toileting, eating, and personal hygiene
  • Assist residents with walking, mobility, positioning, and transfers according to their service plans
  • Safely use approved mobility equipment and mechanical lifts after completing required training and demonstrating competency
  • Provide meal assistance and support residents with dining needs when required
  • Offer companionship, reassurance, conversation, and emotional support
  • Encourage residents to participate in meals, programs, and community activities
  • Respond promptly to resident requests and emergency-call systems
  • Observe residents for changes in physical condition, behavior, cognition, mood, appetite, mobility, or daily routine
  • Promptly report changes or concerns to the licensed nurse or appropriate supervisor
  • Obtain and document vital signs when trained, determined competent, and assigned to do so
  • Follow individualized resident service plans, care instructions, and safety procedures
  • Provide medication reminders and permitted assistance with residents’ self-administration of medications
  • Verify and document self-administration when assigned
  • Assist residents with opening approved medication packaging when authorized
  • Promptly report medication refusals, missed doses, medication concerns, or changes in condition to the licensed nurse
  • Document personal care and services accurately and promptly
  • Maintain clean, safe, and organized resident living areas
  • Assist with personal laundry and light resident-area housekeeping as assigned
  • Promote resident engagement, independence, dignity, choice, and personal preferences
  • Protect resident privacy, rights, personal information, and confidentiality
  • Communicate respectfully with residents, families, visitors, and team members
  • Follow infection-control, resident-safety, emergency, and fall-prevention procedures
  • Participate in required orientation, training, competency reviews, meetings, and in-service education
  • Complete additional resident-care duties permitted by the employee’s training, competency, and assigned role

  • Dependable attendance and punctuality
  • Previous assisted living, memory care, senior care, home care, or healthcare experience is preferred
  • Previous experience assisting with activities of daily living is preferred
  • Experience supporting residents with dementia or memory impairment is preferred
  • Basic computer skills and the ability to complete electronic documentation
  • Current CPR or first-aid certification, if required by the community
  • Ability to stand, walk, bend, kneel, reach, and remain active throughout the workday
  • Ability to safely lift, transfer, push, or pull up to (physical requirement) pounds with assistance or available equipment
  • Ability to successfully complete required background checks, health screenings, training, and employment requirements

Connecticut Role Requirements

  • Must complete all required orientation, training, and competency evaluations
  • May only perform duties permitted by the employee’s training, demonstrated competency, assigned role, resident service plan, and community procedures
  • May provide medication reminders and permitted assistance with residents’ self-administration of medications
  • May not independently administer medications, select doses, alter provider orders, or make medication decisions
  • Must promptly report changes in resident condition and situations requiring nursing judgment
  • May not independently perform nursing assessments, develop nursing care plans, provide nursing diagnoses, or perform duties reserved for licensed nurses
  • Vital signs and other health-related tasks may only be performed after appropriate training, competency verification, and assignment
