Licensed Practical Nurse (LPN) Responsibilities:
Provide skilled nursing care to residents, including administering medications, wound care, and vital sign monitoring
Develop and implement individualized care plans in collaboration with interdisciplinary team members
Monitor and report changes in resident condition to the Nurse Manager/Charge Nurse
Communicate effectively with residents, families, and other healthcare professionals
Participate in quality improvement initiatives to enhance the delivery of care
Maintain accurate and timely documentation of resident care and services
