Contract Dates: 06/22/2026 – 09/20/2026
Schedule: Day Shift | 5x8s | Monday–Friday | No Weekends | No Call
Mileage Reimbursement Provided

Position Overview

As a Home Health Physical Therapist, you will evaluate and treat patients in their homes, helping restore mobility, improve function, and enhance quality of life. You’ll collaborate with interdisciplinary teams while delivering compassionate, evidence-based care.

Key Responsibilities

  • Perform comprehensive patient evaluations and assessments

  • Develop individualized treatment and discharge plans

  • Deliver therapy interventions using appropriate clinical techniques

  • Complete accurate and timely clinical documentation

  • Supervise Physical Therapist Assistants as assigned

  • Collaborate with physicians, nurses, and interdisciplinary teams

RequirementsExperience

  • Minimum 1 year of recent Home Health Physical Therapy experience REQUIRED

  • Must have experience with:

    • HCHB (Homecare Homebase)

    • OASIS documentation

Education

  • Doctor of Physical Therapy (DPT) required for licenses obtained January 2016 or later

Certifications & Licensure

  • Active California Physical Therapy license

  • BLS or CPR certification from an approved provider

  • Valid Driver’s License

  • Must be at least 21 years old
