Salary range: $40.00 - $45.00

40 hours

Monday - Friday with a weekend rotation

This is a 117 bed skilled nursing facility offering long-term and short-term rehab.

Responsibilities

  • Assess patients to determine physical therapy needs and develop individualized plans.

  • Implement evidence-based PT treatments and document progress in notes.

  • Set measurable patient outcomes and track progress against goals.

  • Collaborate with physicians and care teams to refine treatment plans.

  • Maintain accurate documentation for compliance and billing purposes.

  • Contribute to a transparent performance culture with clear metrics and targets.

  • Ensure adherence to safety, infection control, and facility policies.

  • Demonstrate accountability by managing own caseload and achieving personal targets.

  • Educate patients and families on exercises, precautions, and home programs.

  • Identify opportunities for professional development and pursue continuous learning.

  • Participate in on-site team meetings and contribute to care coordination.

  • Assist in pipeline development by identifying potential referrals and community outreach.


Experience/Qualifications

  • Licensed Physical Therapist with current state license and eligibility for supervision where required.

  • Demonstrated experience conducting initial assessments and developing individualized PT plans.

  • Proficiency in evidence-based PT treatments and documentation for progress notes.

  • Accurate documentation for compliance, billing, and audit readiness.

  • Ability to set measurable patient outcomes and track progress against goals.

  • Strong collaboration with physicians and care teams to refine treatment plans.

  • Self-management of caseload with accountability for personal targets and performance.

  • Commitment to safety, infection control, and facility policies.

  • Solid patient and family education skills for home programs and precautions.

  • Willingness to engage in professional development and continuous learning.
