JOB SUMMARY:
The position of Community Health Representative (CHR) will be a supportive member of the Nooksack health Department under the guidance of the Medical Clinic Manager. The position of CHR is the primary link between the Nooksack Indian Tribal Community and the Nooksack Health Department and other health related resources available to access and improve the health awareness and status of the Nooksack Indian Tribal Community Members. The CHR position is equally responsible for the equally assurance and maintenance of confidential client information and assists to maintain health care facilities in compliance with Federal HIPPA and Indian Health Services quality safety standards. To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. In addition, the requirements listed below are representative of the knowledge, skill and/or ability required.
MAJOR TASKS AND RESPONSIBILITIES:
- Responsible to provide health care services to the Nooksack Indian Tribal Community.
- Responsible to document, maintain and complete daily transportation logs and forms.
- Responsible for providing information in regards to resource benefits in the Nooksack Indian Tribal Community and Whatcom County.
- Responsible to report to Supervisor and document any changes and/or updates, or cancellations to daily schedule.
- Responsible for maintaining clear and detail oriented documentation of all client interactions and services.
- Participates in internal process improvement team.
- Provides limited medical transportation services to Nooksack Tribal members without other means of travel to and from various sources of health care facilities for routine non-emergency appointments.
- Provides delivery services to tribal members for medications, medical supplies, and equipment when requested.
- Provides resource information for clients; directs client to correct contact person to answer resource needs and questions.
- Provides simple and basic home chore services on a limited basis and in urgent situations until other more permanent support can be found
- Provide transportation to WIC Clients to Clinic
- Provides health needs to community Elders by providing or arranging visits to Elder in the hospital or nursing home facilities, and conducting home visits upon the Elder to other tribal members return home.
- Responsible to comply with all safety rules and regulations, follows proper safety procedures and utilizes proper protective equipment at all times.
- Must complete and turn in all billing documents for transportation services performed on a daily basis to the medical billers.
OTHER DUTIES:
Because of the Tribe's commitment to community service and the well-being of its members, each employee may be expected to perform a wide range of office and field duties from time to time. Such duties may or may not be related to their regular responsibilities.
PREFERENCE:
- Indian Preference Policy applies to this and all positions with the Nooksack Indian Tribe.
MINIMUM QUALIFICATIONS
The following qualifications are required for the incumbent to have, in order to be considered for the position.
REQUIRED EDUCATION, EXPERIENCE AND TRAINING FOR POSITION:
- High School Diploma or GED required
- Minimum 6 months experience as a Community Health Representative.
REQUIRED SKILLS/KNOWLEDGE/ABILITIES POSITION:
- Must be computer literate with specific work knowledge in Microsoft Office
- Must be able to communicate clearly, effectively and courteously with Nooksack Health Department Staff and supervisor.
- Must continuously maintain a courteous and professional rapport with the clients and staff of the Nooksack Indian Tribal community
- Must be respectful and sensitive to the values and culture of the Nooksack Indian Tribal Community
- Must become or be familiar with the Nooksack Indian Tribal community health departments and available resources available for patients, clients and their families
- Must be caring, empathetic and dependable
- Must be able to work independently with little supervision
- Strong decision making and practical skills
- Must adhere to Federal Privacy Act and comply with Federal HIPPA regulations
REQUIRED CONDITIONS OF EMPLOYMENT:
- Must pass alcohol/drug test at time of hire and throughout employment.
- Must pass criminal background check at time of hire and periodically thereafter.
- Proof of negative TB test within past 12 months
- HIV/AIDS training or obtain within the first month of employment
REQUIRED LICENSES OR CERTIFICATIONS:
- Must have and maintain throughout employment a valid Washington State Driver’s License and meet the insurability requirements of the Tribe. (Must Provide Driving Abstract at time of hire).
- Current Basic Cardiac Life Support (CPR) and Emergency First Aid Certifications (must be obtained within six months of hire and kept current thereafter).
PHYSICAL REQUIREMENTS (The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.):
- Standing/walking: Occasionally-frequently, in combination with walking while performing office duties, indoor on carpeted or tiled flooring and outdoor on pavement, wooded surface, grass, and other sometimes uneven ground
- Sitting: frequently-constantly, on adjustable chair at desk or assigned work area, May stand if desired
- Lift/carry: Occasionally 1-20 lbs.; files, paperwork, supplies. Seldom, up to 35 lbs; boxes of records
- Push/pull: Occasionally, using both arms/hands, required force of 5-20 lbs.; opening doors/file drawers, filing documents.
- Climbing: Seldom, stairs at some locations
- Bending/twisting: Frequently, at waist/knees/neck to and from seated position while working at desk, filing, and records handling.
- Kneeling/crouching/crawling: Not usually required. May kneel or crouch seldom while retrieving items at floor level
- Hands/arms: Constant use of both hands/arms in reaching/handling/grasping/fingering while keyboarding, using phone, calculator, copier, and other office equipment, writing instructions, filing, and performing other administrative duties. Job requirements include forward and over shoulder reaching on a frequent basis
- Keyboarding: frequently for short and long intervals throughout work shift depending on specific work requirements.
- Sight: constantly, in using computers, reading fine print, interacting with the public and coworkers; visual requirements include hand/eye coordination & visual acuity in near/mid ranges.
- Speech/hearing: constantly in answering telephones, communicating with customers, co-workers while giving and receiving instructions.
- Location: work is performed indoors in a climate controlled office environment
DESIRED SKILLS/KNOWLEDGE/ABILITIES POSITION
- Knowledgeable about Nooksack Indian Community members location of homes and be familiar with family names.
- A positive, personable and objective relationship with cliental.
- Knowledge of health resources and to the ability to convey this to all Community members.
- Ability to act as an advocate for community members without consideration of family background or their personal habits.
- Able to work closely with coordinate with the Nurse Practitioner and Community/Public Health Nurse to assure timely referrals, follow-ups and participation as needed.
