We are seeking dedicated and compassionate Licensed Practical Nurses (LPNs) who are passionate about resident-centered care to join our team at Oak Knoll Rehabilitation and Healthcare Center. What You'll Do:

  • Administer medications and treatments per physician orders

  • Monitor resident health and report changes promptly

  • Document and transcribe physician orders accurately

  • Collaborate on care plans and attend interdisciplinary care meetings

  • Supervise and mentor CNAs; ensure completion of care assignments

  • Participate in orientation and training of new nursing staff

  • Ensure compliance with infection control, safety, and resident rights protocols

  • Communicate changes in resident conditions to physicians, supervisors, and families

  • Support facility leadership with audits, assessments, and other nursing functions

Qualifications:

  • Licensed Practical Nurse (LPN) in the state of Massachusetts

  • CPR Certification required

  • 1+ year of experience in geriatric or rehabilitation nursing preferred

  • Excellent written and verbal communication skills

  • Proficient in Microsoft Office and electronic documentation systems

  • Able to lift a minimum of 50 lbs and perform physical nursing tasks

  • Ability to work assigned shifts and adapt to a fast-paced environment
