Jackson, Mississippi.
Responsibilities:
- Evaluate patients and develop individualized treatment plans
- Provide in-home physical therapy services
- Document progress and outcomes accurately
- Coordinate with the care team to ensure optimal patient care
Qualifications:
- Active PT license in the state of Mississippi
- Home health experience preferred
- Strong clinical and interpersonal skills
