PHYSICAL THERAPIST (PT)

Schedule: Permanent (PRN opportunities available)

Compensation:

• Permanent: $60 – $69 per hour

• PRN: $70 – $75 per hour

• Evaluate and treat SNF residents

• Deliver skilled therapy services and documentation

• Ensure Medicare/PDPM compliance

• Collaborate with interdisciplinary care team

• Maintain productivity and quality standards

• Active Physical Therapist (PT) license

• SNF or post-acute rehab experience required

• Knowledge of Medicare/PDPM and documentation standards

• Strong communication and teamwork skills
