Position Overview

As a Physical Therapist, you will evaluate, develop, and implement individualized treatment plans for residents and patients with temporary or long-term physical conditions. You will play a key role in restoring function, promoting healing, and improving overall quality of life.

Key Responsibilities
  • Evaluate patients and develop individualized treatment plans

  • Plan and administer therapeutic interventions to relieve pain and improve mobility

  • Restore or enhance functional movement and independence

  • Ensure all treatments follow the established plan of care

  • Monitor and document patient progress and response to treatment

  • Educate patients, families, and caregivers on therapy plans and goals

  • Maintain accurate documentation in compliance with state and federal regulations

Qualifications
  • Current Physical Therapist (PT) license (state-specific)

  • Graduate of an accredited Physical Therapy program

  • Strong clinical assessment and documentation skills

  • Excellent communication and teamwork abilities

  • Experience in skilled nursing or long-term care preferred (but not required)
