The Lakes Home Care is currently seeking a dedicated and passionate Speech Therapist (Speech-Language Pathologist) to join our growing team. The ideal candidate will assess, diagnose, and treat speech, language, voice, cognitive-communication, and swallowing disorders across various age groups, including neonatal, pediatric, adolescent, adult, and geriatric populations.

Responsibilities:
  • Conduct comprehensive evaluations and assessments of patients with speech, language, cognitive, and swallowing disorders

  • Develop individualized treatment plans based on patient evaluations and diagnosis

  • Provide direct therapeutic interventions using evidence-based practices

  • Monitor and document patient progress, adjusting treatment plans as needed

  • Educate patients and families on communication strategies and home programs

  • Collaborate with physicians, occupational therapists, physical therapists, and other healthcare professionals to ensure holistic care

  • Supervise and support Speech Therapist Assistants as required by state guidelines

  • Implement and train on augmentative and alternative communication (AAC) systems

  • Maintain accurate, timely, and compliant documentation in accordance with professional and regulatory standards

  • Participate in team meetings and contribute to interdisciplinary care planning

Skills and Experience:
  • Experience working with pediatric, adult, and geriatric populations in various settings (home health, schools, hospitals, etc.)

  • Strong understanding of speech-language development, disorders, and intervention techniques

  • Knowledge of IEPs, IFSPs, and documentation procedures in educational settings (if applicable)

  • Familiarity with AAC devices and strategies

  • Ability to adapt treatment to critical care and medically complex environments

  • Excellent communication, collaboration, and case management skills

  • Compassionate and patient-centered approach to care

  • Experience with standardized assessments and outcome measurement tools

Requirements:
  • Master’s Degree in Speech-Language Pathology from an accredited program

  • Current and valid State Licensure or Certification in Speech-Language Pathology

  • Certificate of Clinical Competence (CCC-SLP) from ASHA (or CFY candidates will be considered)

  • CPR certification (if required by state regulations)

  • Ability to pass background check and meet health screening requirements

  • Commitment to continuing education and maintaining licensure
