Experience/Qualifications
Licensed Physical Therapist with current state license required.
Compliance with regulatory guidelines, safety protocols, and ethics.
Demonstrated on-site physical therapy treatment delivery per plan of care.
Accurate documentation of treatments and outcomes in EHR within 24 hours.
Regular progress assessments; adjust plans and record rationale clearly.
Interdisciplinary collaboration for care coordination and discharge planning.
Strong communication skills to educate patients and families on home programs.
Commitment to ongoing professional development and certifications.
Productivity targets with quality-of-care balance; track performance metrics.
Proactive participation in process improvements and performance reviews.
