What You’ll Do

As a Physical Therapist in a Skilled Nursing Facility, you will help patients regain mobility and independence following illness, injury, or surgery.


Key Responsibilities:

  • Evaluate patients and develop individualized treatment plans
  • Provide evidence-based physical therapy interventions
  • Collaborate with physicians, nurses, and rehab team members
  • Supervise Physical Therapy Assistants (PTAs)
  • Document patient progress accurately and timely
  • Participate in care planning and interdisciplinary meetings
  • Educate patients and caregivers on mobility and safety
  • Support discharge planning and functional outcomes

Requirements:

Qualifications

  • Graduate of an accredited Physical Therapy program (DPT, MS, or BS)
  • Active Ohio Physical Therapist license (or eligible)
  • Strong communication and teamwork skills
  • Passion for working with geriatric and post-acute populations
