Spring River Rehabilitation & Care Center is seeking a Registered Dietitian (RD) to join our interdisciplinary team. This role is responsible for overseeing the nutritional care of residents in a skilled nursing and long-term care setting, ensuring compliance with regulatory standards while supporting resident-centered care.
Key ResponsibilitiesComplete nutrition assessments for all new admissions, readmissions, and significant changes
Develop and update individualized nutrition care plans
Monitor weight trends, intake, and nutritional status

Provide recommendations and interventions for:
Weight loss/gain
Wound healing
Malnutrition
Specialized diets

Collaborate with:
Nursing
Providers
Dietary staff
Interdisciplinary team (IDT)

Participate in:
Care conferences
QAPI initiatives
Survey readiness and compliance activities

Ensure compliance with:
CMS regulations
State guidelines
Facility policies

Review and support:
Menu compliance
Therapeutic diet accuracy
Food service practices
Registered Dietitian (RD/RDN) credential required
Licensed in the State of New Mexico (or eligible)
Experience in long-term care or skilled nursing preferred

Strong understanding of:
CMS regulations
Nutrition support in elderly population
Wound care nutrition
