Job Summary:
We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our dedicated team at Nightingale Nursing and Rehab Center in Erie, Pennsylvania. As an LPN, you will play a vital role in providing high-quality patient care and supporting our mission to deliver exceptional healthcare services to our residents.
Responsibilities:
Provide direct patient care to residents in our long-term care facility, including administering medications, monitoring vital signs, and performing routine nursing procedures
Collaborate with interdisciplinary teams to develop and implement individualized care plans tailored to each resident's needs
Conduct routine rounds to ensure resident safety and well-being, and address any concerns or issues promptly
Document patient information accurately and thoroughly in electronic medical records
Participate in quality improvement initiatives to enhance clinical outcomes and patient satisfaction
Qualifications:
Current licensure as a Licensed Practical Nurse (LPN) in the State of Pennsylvania

High school diploma or equivalent required; post-secondary education in nursing or related field preferred
