About the Role:
As a Physical Therapy Assistant at Mohawk Meadows Rehab and Nursing Center, you will work alongside our experienced physical therapists to provide individualized and effective physical therapy interventions to our residents. You will be responsible for assisting with the development and implementation of treatment plans, conducting physical therapy treatments, and maintaining accurate records of resident progress.
Responsibilities:
• Assist with conducting physical therapy treatments, including exercises, evaluations, and assessments
• Assist with the development and implementation of treatment plans in collaboration with the physical therapist
• Provide education and training to residents, families, and caregivers on physical therapy techniques and exercises
• Maintain accurate records of resident progress and treatment plans
• Communicate effectively with the interdisciplinary team to ensure coordinated care
• Participate in quality assurance activities and maintain a safe and clean treatment environment
