Per Diem Dietitian

Job Summary:

Ingraham Manor Rehab and Nursing, a leading provider of rehabilitation and skilled nursing care in Bristol, CT, is seeking a per diem Dietitian to join our team. As a Per Diem Dietitian, you will play a vital role in ensuring the nutritional needs of our patients are met and exceeded. Responsibilities:

• Plan and implement individualized nutrition care plans in collaboration with the interdisciplinary team

• Conduct dietary assessments and develop personalized meal plans for patients with various medical conditions

• Monitor and evaluate patient progress, making adjustments to nutrition care plans as needed

• Provide nutrition education and counseling to patients, families, and healthcare staff

• Develop and maintain accurate and up-to-date records of patient care and nutrition plans

• Collaborate with the food service team to ensure menu plans meet patient needs and dietary restrictions

• Stay current with industry developments, trends, and research to maintain expertise and knowledge

Requirements:

• Bachelor's degree in Nutrition

• Strong communication and interpersonal skills

• Ability to work independently and as part of a team

• Familiarity with electronic medical records (EMRs) and nutrition care software
