Tolstoy Foundation Rehabilitation & Nursing Center is seeking a compassionate, motivated, and resident-focused Occupational Therapist to join our rehabilitation team on a part-time basis.

Located in the heart of Rockland County, Tolstoy is a boutique skilled nursing and rehabilitation center with a home-like atmosphere and a strong commitment to helping residents regain independence and return home whenever possible.

Position Summary

The Occupational Therapist evaluates and treats residents with physical, cognitive, and functional limitations to maximize independence and quality of life.

Responsibilities

• Perform occupational therapy evaluations and develop individualized treatment plans
• Provide skilled occupational therapy interventions
• Assess activities of daily living (ADLs), cognition, safety awareness, and functional mobility
• Develop discharge recommendations and home safety plans
• Collaborate with the interdisciplinary team
• Participate in care plan meetings
• Complete accurate and timely documentation
• Maintain compliance with Medicare, Medicaid, managed care, CMS, and New York State regulations
• Promote resident engagement, independence, and quality of life •COTA supervision: Provide clinical oversight and guidance to Certified Occupational Therapy Assistants (COTAs) as needed

Qualifications

• Graduate of an accredited Occupational Therapy program
• Current New York State Occupational Therapist license in good standing
• Skilled nursing facility and/or rehabilitation experience preferred
• Strong clinical assessment and treatment skills
• Excellent communication and teamwork abilities
• Experience with PointClickCare (PCC), Net Health, NetSmart, or similar therapy documentation systems preferred
• Knowledge of PDPM, Medicare regulations, and current post-acute care trends preferred

• Part-time

• Additional hours may be available based on census and facility needs
