Responsibilities:
Provide direct patient care, including administering medications, wound care, and treatments, under the supervision of a registered nurse or physician
Monitor and report patients' conditions, including changes in vital signs, behavior, and symptoms
Develop and implement individualized care plans in collaboration with the interdisciplinary team
Maintain accurate and detailed records of patient care and treatments
Participate in care conference meetings to discuss patients' progress and concerns
Collaborate with other healthcare professionals to ensure seamless care transitions and communication
