Physical Therapist (PT) Full Time- Loudoun Rehabilitation and Nursing

Loudoun Rehabilitation and Nursing is seeking a dedicated and compassionate Full-Time Physical Therapist (PT) to join our rehabilitation team and provide exceptional care to our residents.

Duties and Responsibilities for Physical Therapist (PT) Full-Time:

  • Perform comprehensive evaluations of patients requiring skilled physical therapy services.

  • Assess patients’ medical conditions, functional abilities, limitations, restrictions, and rehabilitation potential.

  • Develop and implement individualized treatment plans with specific, measurable goals based on each patient’s needs, abilities, and tolerance.

  • Provide skilled therapeutic interventions to improve mobility, strength, balance, safety, and overall functional independence.

  • Monitor patient progress and adjust treatment plans as appropriate to maximize outcomes.

  • Establish realistic and achievable goals in collaboration with patients, families, and the interdisciplinary team.

  • Educate patients and families regarding treatment plans, safety strategies, and techniques to promote continued progress.

  • Complete all required clinical documentation, including evaluations, progress notes, recertifications, and discharge summaries.

  • Collaborate with nursing, occupational therapy, speech therapy, physicians, and other interdisciplinary team members to provide coordinated, patient-centered care.

  • Participate in facility quality initiatives focused on improving clinical outcomes and resident satisfaction.

  • Maintain compliance with HIPAA regulations, confidentiality standards, and Federal Resident Rights.

  • Perform additional duties as assigned by the Director of Rehabilitation.

Requirements and Qualifications for Physical Therapist (PT) Full-Time:

  • Graduate of an accredited Physical Therapy program.

  • Current and valid Physical Therapist license in the state of Virginia.

  • Experience in skilled nursing, long-term care, or geriatric rehabilitation preferred, but not required.

  • Strong clinical reasoning, communication, and organizational skills.

  • Ability to work collaboratively as part of an interdisciplinary team.

  • Commitment to providing compassionate, high-quality patient care.
