Job Summary:

We are seeking a highly motivated and compassionate Registered Dietitian to join our team at Van Duyn Center for Rehabilitation and Nursing. As a Registered Dietitian, you will play a vital role in ensuring the nutritional well-being of our residents, providing exceptional care and support to help them thrive. Responsibilities:

• Coordinate nutritional services for residents, including planning and implementing individualized nutrition plans

• Complete and document nutrition assessments, monitoring residents' progress and adjusting plans as needed

• Participate in the resident care planning process, collaborating with interdisciplinary teams to ensure comprehensive care

• Provide nutritional education and counseling to residents and their families, empowering them to take an active role in their health and wellness

• Demonstrate a commitment to excellent customer service, prioritizing resident satisfaction and well-being

Requirements/Qualifications:

• New York State Registered Dietitian certification

• Rehab experience a plus, but not required

• Ability to work in a fast-paced environment, prioritizing multiple tasks and managing competing demands

• Excellent customer service skills, with a focus on delivering personalized care and support
