Dawn Hill Home for Rehab and Nursing is seeking a Physical Therapist (PT) to join our rehab department in Bristol, RI.

What You’ll Do

• Evaluate residents and develop individualized physical therapy plans of care

• Provide skilled interventions addressing mobility, balance, strength, endurance, pain, fall risk, and functional independence

• Progress residents toward meaningful short- and long-term functional goals

• Supervise and collaborate with PTA staff as appropriate

• Complete accurate, timely, and defensible clinical documentation

• Collaborate with nursing, rehab, providers, and the interdisciplinary team to support coordinated care

• Educate residents, families, and staff regarding mobility, transfers, safety, equipment, and therapeutic exercise

• Participate in discharge planning and recommend appropriate equipment and services for safe transitions

What We’re Looking For

• Graduate of an accredited Physical Therapy program

• Active Rhode Island Physical Therapist license or eligibility for licensure

• Long-term care, skilled nursing, or geriatric experience preferred, but not required

• Strong clinical reasoning, communication, and teamwork skills

• Commitment to resident-centered care and functional outcomes

• New graduates are encouraged to apply
