Key Responsibilities
  • Assess and address residents’ psychosocial, emotional, and mental health needs

  • Complete admissions, care plans, and discharge planning documentation

  • Collaborate with nursing, therapy, and leadership teams to support resident outcomes

  • Advocate for resident rights, dignity, and individualized care planning

  • Provide counseling and support to residents and families

  • Coordinate and maintain community resources and services

  • Ensure timely, accurate documentation in compliance with regulatory standards

  • Support implementation of policies related to social services and resident care

Qualifications
  • Active LSW license in the State of Ohio (REQUIRED)

  • Minimum 2 years of social work experience in long-term care or skilled nursing

  • Strong communication, organizational, and interpersonal skills

  • Ability to work effectively in a fast-paced, team-oriented environment

  • Passion for working with geriatric populations and advocating for resident well-being
