- Deliver skilled physical therapy interventions in the patient's home as delegated by and under the supervision of the supervising Physical Therapist and plan of care
- Assist patients in improving strength, mobility, balance, and functional independence
- Complete visit documentation accurately and on time in compliance with Medicare, ACHC, and agency policies
- Communicate patient progress, changes in status, and any clinical concerns to the supervising PT promptly
- Participate in care coordination with the clinical team and patient's physician as needed
- Active Massachusetts PTA license in good standing
- Graduate of an accredited Physical Therapist Assistant program
- 1+ year of home health or outpatient/rehab experience preferred; strong new grads considered
- CPR/BLS certification (Healthcare Provider level)
- Reliable transportation, valid MA driver's license, and current auto insurance
- Available for at least 2–4 visits per week (flexible on timing)
