Location: Mahoning Post Acute Nursing and Rehab

Job Type: Full Time

We are seeking a dedicated and compassionate Occupational Therapist (OT) to join our interdisciplinary team in a skilled nursing facility. The ideal candidate is committed to improving the quality of life for older adults through evidence‑based, person‑centered care. This role involves evaluating residents, developing individualized treatment plans, and delivering therapeutic interventions that promote independence, safety, and functional ability.

Clinical Care
  • Conduct comprehensive evaluations of residents’ physical, cognitive, and psycho-social abilities.

  • Develop individualized treatment plans focused on functional improvement, ADLs, mobility, and safety.

  • Provide direct therapy services including therapeutic exercises, ADL retraining, cognitive interventions, and adaptive equipment training.

  • Recommend and train residents on the use of assistive devices and adaptive equipment.

  • Document evaluations, progress notes, and discharge summaries in accordance with regulatory and facility standards.

Interdisciplinary Collaboration
  • Communicate treatment goals and progress with residents, families, and caregivers.

  • Provide education on safe mobility, fall prevention, and home safety strategies.

Regulatory & Administrative
  • Maintain accurate and timely documentation for billing and reimbursement.

  • Adhere to facility policies, professional ethics, and best‑practice standards.

Qualifications
  • Current state OT license

  • Strong clinical reasoning and communication skills.

  • Experience in a skilled nursing facility or geriatric setting preferred but not required.

  • Ability to work independently and as part of a collaborative team.
