Part‑Time LPN to provide focused home‑health visits for patients with

Key Duties
- Complete home‑health nursing visits ($30 per visit)
- Focus on hypertension and diabetic patient care
- Perform assessments, vitals, medication teaching
- Provide disease‑management education
- Document visits accurately
- Communicate patient changes to RN Case Manager
Requirements
- Active LPN license in Massachusetts
- Home‑health experience preferred
- Strong patient‑education and communication skills
- Reliable, professional, and able to work independently
