Position SummaryThe PRN Physical Therapist is responsible for evaluating residents, developing individualized treatment plans, and providing skilled therapy services to promote functional recovery and improve quality of life. This role works closely with residents, families, and the interdisciplinary care team to deliver effective, resident-centered rehabilitation.
Key ResponsibilitiesResident Evaluation & TreatmentPerform comprehensive evaluations based on physician referrals and resident needs.
Develop individualized treatment plans with measurable, functional goals.
Provide skilled one-on-one physical therapy interventions to improve mobility, strength, balance, endurance, and independence.
Monitor resident progress and modify treatment plans as needed to achieve optimal outcomes.
Participate in discharge planning and provide education to residents and families regarding safety and continued care.
Complete timely and accurate documentation in accordance with professional standards, facility policies, and regulatory requirements.
Maintain appropriate treatment records and billing documentation.
Stay current with Medicare guidelines, reimbursement requirements, and industry best practices.
Collaborate with physicians, nursing staff, occupational therapists, speech therapists, and other interdisciplinary team members to coordinate resident care.
Participate in care conferences, quality improvement initiatives, and team meetings.
Communicate effectively with residents, families, and healthcare professionals to support positive outcomes.
Supervise Physical Therapist Assistants (PTAs), rehabilitation technicians, and students as appropriate.
Provide guidance and support related to restorative techniques and safe mobility practices.
Assist with staff education, in-services, and professional development activities.
Help maintain a clean, organized, and safe therapy environment.
Ensure therapy equipment is properly maintained and report any concerns.
Provide coverage at nearby facilities as needed based on resident care needs.
Graduate of an accredited Physical Therapy program.
Current and valid Physical Therapist license in the state of practice.
Experience in skilled nursing, rehabilitation, or long-term care is preferred but not required.
Strong clinical judgment, communication skills, and ability to work collaboratively.
Compassionate and patient-focused approach to care.
Ability to safely assist residents with mobility, including lifting up to 50 pounds when necessary.
Ability to work independently while contributing to a team-oriented environment.
Active, hands-on rehabilitation setting with regular interaction with residents, families, and healthcare team members.
Ability to stand, walk, bend, stoop, push, and pull throughout the workday.
Ability to assist with resident transfers and mobility activities.
Potential exposure to blood, bodily fluids, and infectious diseases while following appropriate safety protocols.
