Cornerstone Rehab is seeking an experienced and motivated Senior Physical Therapist to oversee clinical responsibilities across multiple facilities. Responsibilities:Provide physical therapy to adult and geriatric patients.Prepare documentation, progress notes, and discharge summaries.Establish a plan of care in accordance with patients' needs and capabilities.Travel to nearby centers as neededQualifications:Valid State Physical Therapist License and CredentialsSNF and/or Acute care experienceCornerstone Rehab delivers innovative rehab management solutions to skilled nursing facilities in the United States.
