As an LPN at our Skilled Nursing Facility, you will play a vital role in delivering high-quality patient care, promoting independence, and enhancing the overall well-being of our residents.
Responsibilities
Provide direct patient care to residents in our Skilled Nursing Facility, including administering medications, wound care, and other treatments as prescribed by physicians
Monitor and report changes in patients' conditions to the nursing team, physicians, and family members
Collaborate with other healthcare professionals to develop and implement individualized care plans
Educate patients and their families on self-care techniques, medication management, and disease prevention
Participate in ongoing quality improvement initiatives to ensure the highest level of patient care
