What You'll DoProgram Coordination & Person-Centered Services
  • Coordinate and oversee community-based services for individuals receiving NJ DDD supports.

  • Conduct intake assessments and ensure all required documentation is completed accurately and on time.

  • Develop, implement, and monitor individualized, person-centered service plans based on each individual's strengths, goals, and support needs.

  • Coordinate referrals to community resources, healthcare providers, employment programs, and other support services.

  • Assist individuals in accessing community activities, appointments, employment opportunities, and meaningful experiences that promote independence and inclusion.

Leadership & Team Support
  • Supervise, mentor, train, and support Direct Support Professionals (DSPs) and program staff.

  • Promote a collaborative, respectful, and person-centered team culture.

  • Conduct performance evaluations and provide ongoing coaching to support staff development and success.

  • Work closely with leadership to strengthen program operations and improve service delivery.

Compliance & Quality Assurance
  • Ensure compliance with New Jersey DDD regulations, Medicaid requirements, and agency policies.

  • Maintain accurate client records, progress notes, incident reports, and service documentation.

  • Utilize the NJ DDD iRecord system and other electronic documentation platforms to maintain compliance.

  • Monitor service quality, identify opportunities for improvement, and support quality assurance initiatives.

Advocacy & Community Engagement
  • Build strong relationships with individuals served, families, guardians, Support Coordinators, and community partners.

  • Advocate for individuals to ensure they receive the resources and services needed to achieve their goals.

  • Collaborate with interdisciplinary teams to provide coordinated, high-quality care and support.

What We're Looking ForRequired Qualifications
  • Experience working with individuals with developmental disabilities in a community-based or healthcare setting.

  • Strong knowledge of New Jersey DDD regulations, community-based services, and person-centered planning.

  • Experience coordinating services, managing referrals, and maintaining accurate client documentation.

  • Strong leadership, organizational, communication, and problem-solving skills.

  • Ability to supervise, mentor, and motivate staff while maintaining high standards of service.

  • Ability to effectively manage multiple priorities in a fast-paced environment.

Preferred Qualifications
  • Bachelor's degree in Social Work, Human Services, Psychology, Behavioral Health, Healthcare Administration, or a related field.

  • Experience in case management, program coordination, developmental disabilities, or behavioral health services.

  • Experience using the NJ DDD iRecord system or similar electronic documentation platforms.

  • Knowledge of quality assurance, compliance monitoring, and program improvement initiatives.
