What You'll DoProgram Coordination & Person-Centered ServicesCoordinate and oversee community-based services for individuals receiving NJ DDD supports.
Conduct intake assessments and ensure all required documentation is completed accurately and on time.
Develop, implement, and monitor individualized, person-centered service plans based on each individual's strengths, goals, and support needs.
Coordinate referrals to community resources, healthcare providers, employment programs, and other support services.
Assist individuals in accessing community activities, appointments, employment opportunities, and meaningful experiences that promote independence and inclusion.
Supervise, mentor, train, and support Direct Support Professionals (DSPs) and program staff.
Promote a collaborative, respectful, and person-centered team culture.
Conduct performance evaluations and provide ongoing coaching to support staff development and success.
Work closely with leadership to strengthen program operations and improve service delivery.
Ensure compliance with New Jersey DDD regulations, Medicaid requirements, and agency policies.
Maintain accurate client records, progress notes, incident reports, and service documentation.
Utilize the NJ DDD iRecord system and other electronic documentation platforms to maintain compliance.
Monitor service quality, identify opportunities for improvement, and support quality assurance initiatives.
Build strong relationships with individuals served, families, guardians, Support Coordinators, and community partners.
Advocate for individuals to ensure they receive the resources and services needed to achieve their goals.

Collaborate with interdisciplinary teams to provide coordinated, high-quality care and support.
Experience working with individuals with developmental disabilities in a community-based or healthcare setting.
Strong knowledge of New Jersey DDD regulations, community-based services, and person-centered planning.
Experience coordinating services, managing referrals, and maintaining accurate client documentation.
Strong leadership, organizational, communication, and problem-solving skills.
Ability to supervise, mentor, and motivate staff while maintaining high standards of service.
Ability to effectively manage multiple priorities in a fast-paced environment.
Bachelor's degree in Social Work, Human Services, Psychology, Behavioral Health, Healthcare Administration, or a related field.
Experience in case management, program coordination, developmental disabilities, or behavioral health services.
Experience using the NJ DDD iRecord system or similar electronic documentation platforms.
