About the Role:
As an LPN Licensed Practical Nurse at Spring Creek Healthcare Center, you will be responsible for providing high-quality, patient-centered care to our residents. Your weekend shifts will be a critical time for our team, ensuring seamless continuity of care and attention to our residents' unique needs. Your responsibilities will include:
Providing direct patient care, including medication administration, wound care, and vital sign monitoring
Developing and implementing individualized care plans in collaboration with the interdisciplinary care team
Assisting with activities of daily living, such as bathing, dressing, and grooming
Maintaining accurate and detailed records of patient care and services
Collaborating with other healthcare professionals to ensure effective communication and coordination of care
