Position Summary
Nancy Hart Nursing Center is seeking compassionate and motivated Physical Therapists (PT) and Physical Therapist Assistants (PTA) to join our rehabilitation team on a per diem basis. Our therapy team is dedicated to helping residents regain strength, mobility, independence, and overall functional abilities in a skilled nursing and rehabilitation setting.
The PT and PTA will work collaboratively with an interdisciplinary team to provide high-quality, patient-centered rehabilitative care focused on improving residents’ quality of life and functional outcomes.
Responsibilities
Physical Therapist (PT)
- Evaluate residents and develop individualized physical therapy treatment plans
- Establish functional goals and appropriate treatment interventions
- Provide skilled physical therapy services including therapeutic exercise, gait training, balance training, transfer training, and pain management techniques
- Supervise and direct Physical Therapist Assistants in accordance with state practice guidelines
- Assess resident progress and modify treatment plans as appropriate
- Complete evaluations, progress notes, recertifications, and discharge summaries accurately and timely
- Collaborate with physicians, nursing staff, therapy team members, and interdisciplinary departments
Physical Therapist Assistant (PTA)
- Carry out treatment plans developed by the supervising Physical Therapist
- Provide therapeutic exercises, mobility training, gait training, transfer training, and balance activities
- Monitor and report resident responses and progress to the supervising therapist
- Document treatment sessions and resident progress in accordance with facility standards and regulatory requirements
- Educate residents and caregivers on exercises, safety techniques, and mobility strategies
- Maintain a safe and supportive treatment environment
Qualifications
Physical Therapist (PT)
- Current Georgia Physical Therapist license in good standing
- Graduate of an accredited Physical Therapy program
- Skilled nursing or rehabilitation experience preferred
Physical Therapist Assistant (PTA)
- Current Georgia Physical Therapist Assistant license in good standing
- Graduate of an accredited PTA program
- Skilled nursing or rehabilitation experience preferred
Additional Requirements
- Strong communication and interpersonal skills
- Ability to work effectively within a multidisciplinary team
- CPR certification preferred
