MDS/Care Plan Coordinator

Job Summary:

As a MDS/Care Plan Coordinator at Oak Park Oasis, you will play a vital role in ensuring seamless patient care by creating and implementing comprehensive care plans for our residents. With a focus on reducing re-hospitalization rates and promoting independence, you will collaborate with our multidisciplinary team to develop individualized plans that meet each patient's unique needs. Responsibilities:

  • Coordinate with healthcare professionals to develop and implement Individualized Care Plans (ICPs) for each patient, ensuring compliance with Medicare and Medicaid regulations

  • Conduct comprehensive assessments and evaluations to identify patient needs and develop strategies for meeting those needs

  • Collaborate with multidisciplinary teams to develop and update care plans, ensuring continuity of care and effective communication with patients, families, and healthcare providers

  • Monitor patient progress and adjust care plans as necessary to ensure optimal outcomes

  • Coordinate with outside services, such as home health care and durable medical equipment, to facilitate a smooth transition back to the patient's home

  • Maintain accurate and detailed records of patient assessments, care plans, and progress

  • Participate in quality improvement initiatives and maintain confidentiality and adhere to HIPAA guidelines

Requirements:

  • 2 years of experience in a related field, with a strong understanding of MDS/Care Plan coordination principles and practices

  • Licensed or Registered Nurse in the State of Illinois (unencumbered)

  • High school diploma or equivalent required; associate's or bachelor's degree in a related field (e.g., healthcare administration, rehabilitation science, or nursing) preferred

  • Strong analytical and communication skills, with the ability to work effectively with patients, families, and healthcare professionals

  • Ability to adapt to changing priorities and deadlines in a fast-paced environment

  • Basic computer skills and proficiency in electronic medical records (EMRs)
