Job Summary:

We are looking for an experienced and certified Occupational Therapist to care for our patients in a skilled nursing facility setting. As an Occupational Therapist at Samaritan Nursing and Rehab, you will be responsible for providing high-quality occupational therapy services to our patients, including assessing their needs, developing and implementing treatment plans, and educating patients and their families on prevention and healthy habits.

Responsibilities:

• Conduct initial assessments and evaluations to identify patient needs and develop individualized treatment plans
• Implement and deliver occupational therapy services, including exercises, activities, and training to help patients achieve their goals
• Document patient progress and communicate with other healthcare team members to ensure comprehensive care
• Educate patients, families, and caregivers on proper techniques and strategies to promote independence and safety
• Participate in interdisciplinary team meetings to discuss patient care and develop treatment plans
• Maintain accurate and timely records of patient care and services provided

Requirements:

• Master's degree in Occupational Therapy from an accredited program
• Current Wisconsin Occupational Therapy license
• Minimum of 1-2 years of experience as an Occupational Therapist
• Excellent communication and interpersonal skills
• Compassionate and empathetic approach to patient care
• Ability to work independently and as part of a team
• Strong organizational and time management skills
