St. George Village is seeking an experienced and dependable Licensed Practical Nurse to provide leadership and exceptional resident care within our skilled nursing neighborhood. The ideal candidate is assertive, takes initiative, demonstrates strong clinical judgment, and has experience supervising nursing assistants in a skilled nursing environment.
Position SummaryUnder the direct supervision of the Assistant Director of Health Services and the indirect supervision of the Director of Nursing and Health Services, the LPN coordinates and manages resident care. Responsibilities include resident assessments, medication administration, care planning, staff supervision, regulatory compliance, and communication with residents, families, physicians, and other healthcare providers.
Essential ResponsibilitiesAdminister medications and treatments as ordered by physicians and document them according to established policies and regulatory requirements.
Monitor residents for changes in condition and promptly notify the appropriate clinical personnel.
Complete required resident assessments and maintain accurate, timely documentation.
Develop, implement, and update individualized nursing and CNA care plans.
Complete nursing assessments for new residents moving into the skilled nursing neighborhood.
Communicate relevant assessment findings and care needs to team members.
Supervise and provide direction to Certified Nursing Assistants during the assigned shift.
Assign resident-care responsibilities and monitor CNA rounds and task completion.
Delegate staffing assignments to ensure residents receive appropriate and timely care.
Assist with scheduling and resource allocation as needed.
Collaborate with physicians, therapy providers, pharmacies, hospice agencies, laboratories, dietitians, private caregivers, and other healthcare partners.
Coordinate with the pharmacy regarding medication refills, changes, and urgent orders.
Conduct medication administration record audits as assigned.
Complete required weekly, monthly, and quarterly nursing assessments.
Serve as a liaison among residents, families, physicians, responsible parties, and members of the care team.
Provide relevant resident information for quarterly care-planning meetings.
Monitor meal service and collaborate with dietary staff to identify residents who may be at nutritional risk.
Assist with the orientation, training, and mentoring of new care partners.
Collaborate with the Nurse Manager regarding CNA performance, coaching, and corrective action.
Identify opportunities for improvement and assist with developing appropriate action plans.
Follow all safety, infection-control, confidentiality, and regulatory requirements.
Report safety hazards and equipment malfunctions to the appropriate personnel.
Attend required meetings, educational programs, and training sessions.
Perform other duties as assigned.
This position provides shift supervision and direction to approximately two to six non-exempt employees, primarily Certified Nursing Assistants. Approximately 50% of the position’s responsibilities involve supervision and leadership.
QualificationsCurrent, unrestricted Georgia LPN license.
Current CPR certification.
Minimum of two years of related nursing experience.
Previous skilled nursing experience is required.
Experience supervising or directing CNAs is strongly preferred.
Strong medication-administration, documentation, assessment, and care-planning skills.
Demonstrated leadership ability, initiative, confidence, and sound clinical judgment.
Ability to recognize changes in residents’ conditions and respond appropriately.
Excellent communication, interpersonal, and customer-service skills.
Ability to read, write, and follow written and verbal instructions in English.
Ability to interpret policies, safety rules, procedural manuals, and clinical instructions.
Ability to prepare reports and communicate effectively with residents, families, employees, and healthcare providers.
Ability to solve practical problems and work effectively in situations with multiple priorities.

Schedule RequirementsThis is a part-time, night-shift position. Candidates must be willing to work a flexible schedule based on resident-care needs, including weekends and holidays as assigned.
