About the Role:

As a Licensed Practical Nurse (LPN) at Clarion Nursing and Rehab, you will play a crucial role in delivering high-quality care to our residents. Your primary responsibilities will include:

  • Providing direct patient care, including administering medications, wound care, and other medical treatments

  • Conducting vital sign checks, monitoring resident condition, and reporting any changes to the healthcare team

  • Assisting with admitting and discharging residents, ensuring a smooth transition

  • Maintaining accurate and up-to-date resident records and reports

  • Participating in care plan development and implementation

  • Collaborating with other healthcare professionals to ensure comprehensive care

Requirements:

  • Must be a graduate of an accredited LPN program and hold a current LPN license in the state of Pennsylvania

  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams

  • Ability to prioritize tasks, manage time effectively, and maintain a high level of focus in a fast-paced environment

  • Possess a strong commitment to providing exceptional patient care and a passion for delivering compassionate service
