Weekend LPN - Allaire Rehab and Nursing
Job Summary:
As a Weekend LPN at Allaire Rehab & Nursing, you will be part of a dynamic team of healthcare professionals dedicated to providing exceptional care to our residents. Weekend LPN is an integral part of our weekend team, providing essential care and support to our residents who require ongoing medical attention.
Responsibilities:
Provide direct patient care, including administering medications, assessing vital signs, and performing treatments as prescribed by physicians
Develop and implement individualized care plans in collaboration with the healthcare team
Monitor and report changes in patient condition to the Charge Nurse and other healthcare team members
Assist with admissions, discharges, and transfers as needed
Maintain accurate and up-to-date patient records
Collaborate with other departments, such as therapy and dietary, to ensure comprehensive care
Demonstrate strong communication and interpersonal skills, providing exceptional customer service to residents, families, and coworkers
