Job Description:
We are seeking a Physical Therapist for a home health setting. The PT will provide in-home therapy services, develop individualized treatment plans, and collaborate with interdisciplinary care teams to support patient recovery and independence.
Requirements:
• Active Physical Therapist license
• Home health experience preferred
• Strong documentation and time management skills
