Licensed Practical Nurse (LPN) Job Qualifications for Adult Day Center:

  • Current and active license to practice as a Licensed Practical Nurse (LPN) in the state of Wisconsin.

  • CPR and First Aid certification (or willingness to obtain).

  • Strong interpersonal skills with the ability to engage participants, families, and staff in a positive manner.

  • Comfortable assisting with recreational and social activities, including community outings.

  • Ability to work both independently and collaboratively in a team environment.

  • Must be dependable, organized, and adaptable to changing needs throughout the day.

Licensed Practical Nurse (LPN) Job Overview:

The Licensed Practical Nurse (LPN) at our Adult Day Center plays a vital role in supporting the health, safety, and well-being of participants in a dynamic, community-based setting. Unlike traditional nursing roles, our LPNs do a little bit of everything: providing basic medical care, leading activities, assisting with social support, accompanying participants on outings, and helping foster a safe and engaging environment.

This position is ideal for an energetic, flexible, and compassionate nurse who enjoys building relationships with participants and working as part of a team in a lively day program.


Key Responsibilities:

Nursing & Health Support

  • Assist/Formulate care plans for participants.

  • Administer prescribed medications and injections as directed by physicians.

  • Monitor participant health status, report concerns, and communicate with families and healthcare providers as needed.

  • Document all participant care, interventions, and health-related updates accurately.

  • Respond to emergencies and provide first aid when necessary.

Program & Participant Engagement

  • Assist in planning and leading daily activities to promote socialization, wellness, and cognitive engagement.

  • Participate in and supervise community outings, ensuring participant safety.

  • Support participants in maintaining independence, dignity, and quality of life.

Teamwork & Coordination

  • Collaborate with program staff, social workers, and families to meet the holistic needs of participants.

  • Provide guidance and support to staff regarding participant care needs.

  • Maintain compliance with state and federal regulations governing adult day services.
