Position Summary:

As an Early Intervention Service Coordinator, you will conduct comprehensive assessments through observation, interviews, and record review to identify child and family needs. You will play a key role in connecting families with essential services, strengthening early childhood outcomes, and ensuring every family receives the support they deserve. Must be bilingual in English and Spanish to support communication with diverse community members.

Key Responsibilities:

• Build positive, collaborative relationships with families, caregivers, and team members while upholding the organization’s mission and values

• Conduct child and family interviews to assess developmental, social, and environmental needs

• Connect families with appropriate community resources and service providers

• Complete required reports and data submissions in alignment with grant and program guidelines

• Participate in child‑find activities to identify service gaps and support early intervention efforts

• Stay current with best practices, trends, and innovations in case management and service coordination

• Follow all agency safety and infection‑control procedures

• Maintain professional standards regarding attendance, appearance, and conduct

• Perform additional related duties as needed to support program success

Job Competencies & Minimum Qualifications:

• Bachelors degree in health related discipline; RN or BSN; MSW preferred; or NJ certified teacher with Special Education certification

• 2+ years experience working with children (birth to 5 years) and families; prior home visit and case management experience preferred

• Strong computer skills

• Excellent verbal and written communication skills

• Strong organizational and interpersonal skills

• Valid NJ driver's license

• Bilingual in English and Spanish required

In compliance with New Jersey's Pay Transparency Act, the compensation range for this position is USD $26.00 to USD $34.00.
