Position Overview

Spring River Rehabilitation & Care Center is seeking compassionate, dependable, and resident-focused Certified Nursing Assistants (CNAs) to join our skilled nursing team.

As a CNA, you will provide direct, hands-on care to residents while assisting with activities of daily living, mobility, personal hygiene, nutrition, and other individualized needs. Working under the direction of licensed nursing staff, you will play an essential role in promoting resident dignity, independence, safety, comfort, and overall well-being.

Our CNAs are often the team members who spend the most time directly with residents. Your ability to build relationships, recognize changes in condition, communicate concerns, and provide responsive care is critical to achieving positive resident outcomes.

Key Responsibilities
  • Assist residents with activities of daily living (ADLs), including bathing, dressing, grooming, oral hygiene, toileting, eating, and personal care

  • Safely assist residents with transfers, ambulation, mobility, and repositioning in accordance with individualized care plans

  • Utilize mechanical lifts, gait belts, wheelchairs, and other resident mobility equipment appropriately and according to training and facility procedures

  • Accurately obtain, record, and report vital signs, including temperature, pulse, respirations, and blood pressure, as assigned

  • Observe residents for changes in physical, cognitive, emotional, behavioral, or functional condition and promptly report concerns to the supervising nurse

  • Respond promptly, respectfully, and professionally to resident call lights and requests for assistance

  • Provide toileting and incontinence care while maintaining resident privacy, dignity, and comfort

  • Assist residents with meals, hydration, feeding, and nutritional needs according to individualized care plans

  • Reposition residents as directed to promote comfort, mobility, and skin integrity

  • Assist with restorative nursing programs and mobility activities as directed and within the CNA's role

  • Make beds, change linens, and maintain resident rooms and care areas in a clean, safe, and organized condition

  • Accurately document assigned resident care and services in accordance with facility procedures

  • Follow infection prevention practices, including appropriate hand hygiene, use of personal protective equipment (PPE), and transmission-based precautions

  • Follow established fall-prevention, elopement-prevention, resident-safety, and emergency procedures

  • Communicate professionally with residents, families, nurses, coworkers, and members of the interdisciplinary care team

  • Immediately report accidents, incidents, injuries, allegations, abuse or neglect concerns, safety concerns, and significant changes in resident condition according to facility policy

  • Protect resident rights, privacy, dignity, confidentiality, preferences, and personal belongings

  • Follow all Spring River Rehabilitation & Care Center policies and applicable federal and New Mexico requirements governing skilled nursing and resident care

Qualifications
  • Current Certified Nurse Aide (CNA) certification/status meeting applicable New Mexico requirements

  • Must remain eligible and in good standing as required for employment

  • Previous experience in skilled nursing, long-term care, rehabilitation, assisted living, or another healthcare setting preferred; qualified new CNAs are encouraged to apply

  • Compassionate and respectful approach to caring for older adults and residents with varying physical, cognitive, and behavioral needs

  • Strong communication and interpersonal skills

  • Ability to observe and appropriately report changes in resident condition

  • Ability to follow individualized care plans, nursing instructions, and established safety procedures

  • Strong teamwork and collaboration skills

  • Dependable attendance and commitment to assigned schedules

  • Ability to prioritize resident needs and remain organized in a fast-paced healthcare environment

Physical Requirements
  • Ability to assist residents with transfers, repositioning, ambulation, mobility, and activities of daily living

  • Ability to lift, push, pull, and move residents or equipment using appropriate assistance and mechanical devices when required

  • Ability to stand, walk, bend, reach, squat, and perform resident-care activities for extended periods

  • Ability to safely operate resident transfer and mobility equipment following appropriate training

  • Ability to respond promptly and effectively to resident needs, changes in condition, and emergency situations
