Job Summary

Valley Village Care Center is seeking a compassionate and skilled licensed Physical Therapist (PT) to join our rehabilitation team. The Physical Therapist will provide high-quality, patient-centered rehabilitation services to our residents while collaborating with nursing, physicians, and other members of the interdisciplinary team.

Responsibilities

• Conduct comprehensive physical therapy evaluations and assessments.

• Develop and implement individualized plans of care based on each resident’s needs and goals.

• Provide skilled physical therapy interventions to improve mobility, strength, balance, safety, and functional independence.

• Monitor resident progress and modify treatment plans as clinically appropriate.

• Provide resident, family, caregiver, and staff education and training.

• Collaborate with physicians, nursing, occupational therapy, speech therapy, and other members of the interdisciplinary team.

• Maintain accurate and timely documentation in accordance with facility, state, federal, and payer requirements.

• Follow professional standards of practice and applicable regulatory requirements.

Requirements

• Current and active California Physical Therapist (PT) license required.

• Graduate of an accredited Physical Therapy program.

• Must meet all applicable California requirements to practice as a Physical Therapist.

• Strong clinical, communication, and interpersonal skills.

• Ability to work effectively as part of an interdisciplinary team.

• Skilled nursing/post-acute care experience preferred, but new graduates are welcome to apply.
