Position Summary


Under the direction of the Nursing Director, the Licensed Practical Nurse (LPN) coordinates medical care and provides clinical oversight for individuals diagnosed with intellectual and developmental disabilities (IDD) and/or dual diagnoses in residential and day-support settings.

The LPN is responsible for completing and overseeing medical documentation, monitoring the quality and continuity of medical services, and coordinating with direct-care staff who provide medical-related services and assistance.


This is a part-time position requiring strong clinical judgment, organization, documentation, and knowledge of DBHDD standards.


General Medical Oversight


  • Assess, plan, develop, implement, and evaluate interventions that promote individuals’ health, safety, and overall well-being.
  • Participate in the pre-admission process and evaluation of new referrals.
  • Complete nursing assessments for newly admitted individuals as assigned.
  • Collaborate with medical providers, family members, legal representatives, guardians, and other members of the care team to obtain comprehensive medical histories.
  • Identify and prioritize health concerns and intervene appropriately during urgent or crisis situations.
  • Complete accurate and timely progress notes to clearly document nursing services and interventions provided.
  • Establish and supervise general medical triage procedures for residential programs.
  • Coordinate medical appointments, hospital visits, emergency-room care, and follow-up services.
  • Coordinate transportation to medical appointments when required, using a personal or company vehicle as appropriate.
  • Submit a monthly calendar/schedule of medical appointments and in-home nursing services for CRA programs to the Executive Director.
  • Attend Individual Service Plan (ISP) and Supports Intensity Scale (SIS) meetings as assigned.
  • Assist with new-individual intake and transition processes.


Medical Management, Auditing & Reporting


  • Educate, train, and monitor staff to ensure medical services identified in the Individual Service Plan (ISP), Behavior Support Plan (BSP), and other clinical assessments are carried out accurately and within required timeframes.
  • Conduct ongoing assessments of individuals’ health status to facilitate timely interventions and appropriate referrals to healthcare providers.
  • Update Health Risk Screening Tool (HRST) reports at least 90 days before an individual’s birthday and whenever significant physical, mental-health, or medication changes occur.
  • Submit completed HRST documentation to appropriate program management for distribution and implementation.
  • Receive, authenticate, and properly document individual medications and medication orders.
  • Conduct, at minimum, monthly medical audits to verify appropriate medical care, treatment, documentation, and follow-up.
  • Audit Medication Administration Records (MARs) for accuracy and completeness.
  • Monitor medical appointments, required follow-ups, laboratory results, vital signs, blood pressure, weight, bowel-management protocols, medication supplies, and other prescribed health interventions.
  • Monitor the proper storage, security, handling, and disposal of medications.
  • Ensure staff follow established procedures for medication administration and assistance with self-administration.
  • Review records monthly to verify receipt of laboratory reports and completion of appropriate follow-up.
  • Follow established medication-reconciliation protocols for new orders, medication changes, and existing orders.
  • Prepare required quarterly infection-control reports.
  • Submit required nursing, medical, compliance, and quality-assurance reports within established deadlines.
  • Perform other nursing and clinical duties as assigned.

Medication Training & Quality Assurance


  • Provide and/or coordinate ongoing medical-management training for staff, including applicable SSAM and proxy-caregiver training, at least quarterly or as required.
  • Review and assist with developing corrective-action strategies related to medical incidents, medication errors, documentation deficiencies, and other clinical concerns.
  • Participate in organizational committees as assigned, which may include Quality Assurance, Human Rights, Executive Team, and other committees designated by leadership.
  • Meet continuing-education requirements established by DBHDD and Lowe’s Guardian Angel.
  • Maintain comprehensive knowledge of Georgia Department of Behavioral Health and Developmental Disabilities (DBHDD) requirements related to the physical healthcare and safety of individuals receiving services.
  • Promote continuous quality improvement and regulatory compliance across residential and day-support programs.


Qualifications


  • Current and unrestricted Licensed Practical Nurse (LPN) license in Georgia.
  • Current CPR/BLS certification.
  • Experience working with individuals with intellectual/developmental disabilities, behavioral health needs, or dual diagnoses strongly preferred.
  • Knowledge of DBHDD standards, HRST, MAR documentation, medication management, and clinical auditing preferred.
  • Strong clinical documentation, communication, organizational, and time-management skills.
  • Ability to work independently while collaborating effectively with program leadership and direct-care staff.
  • Valid driver’s license, reliable transportation, and appropriate automobile insurance when travel is required.
